Request an Appointment with a Beaverton Dentist

Send us a few details and our team will follow up to find a time that suits your family. Children, teens and adults are all welcome.

Tell us about your family

Please don’t include private medical details in this form — we’ll talk through everything by phone. For a dental emergency, call (503) 822-0096.

Short answer: Fill out the short form on this page with your name, contact details and who the visit is for, and our team will get back to you to find a time. For dental pain, swelling or an injury, please call (503) 822-0096 instead of waiting for a reply.

Tell us a little, and we’ll take it from there

Requesting a visit should take a few minutes. Here is what to include so we can help quickly, and what happens after you send it.

    What to include

    • The patient’s first name and age (or date of birth if you prefer to provide it)
    • Whether the visit is for a child, a teen or an adult
    • A phone number and email where we can reach you
    • Days or times that generally work, if you have a preference
    • A brief reason for the visit, such as a first checkup, a cleaning or a specific concern
    • Whether you would like Dr. Hsu or Dr. Gvozden

    What happens next

    1. We review your request. A member of our team reads it and matches it with the right dentist.
    2. We reach out. We’ll contact you to suggest appointment times and answer questions, including how to prepare.
    3. You come in. For a first visit with a child, our first visit guide can help you get ready. Adults can find details on our new patients page.

Have dental pain, swelling, a knocked-out tooth or a mouth injury? Please call (503) 822-0096 right away. Our emergency dentist page explains what to do in the meantime.

    A note on privacy

Please keep private health details out of the form. There’s no need to describe diagnoses, medications or medical history here; we will talk through anything like that with you directly. Your name, contact details and a short reason for the visit are plenty. For more on how we handle information, see our privacy policy.

Prefer to talk? Call (503) 822-0096 or find us on our contact page. If you have questions about payment or benefits first, our insurance and payment options page is a good place to start.

    Who is this visit for?

Not sure how to fill out the form? Here’s a quick guide.

For a baby, child or teen

Choose a pediatric visit. Dr. Hsu sees patients from infancy through the teen years. Good reasons to book include a first tooth, a first checkup, a regular cleaning or a parent’s concern. Start with pediatric dentistry.

    For an adult

Dr. Gvozden sees adults for exams, cleanings, fillings, crowns, extractions, implants and more. Browse general dentistry to see what fits.

If you are a parent who is nervous yourself

It’s okay to say so in the message. Something like “my child is very shy” or “we’ve had a hard visit before” helps us plan a slower, more comfortable first appointment.

If you are unsure what you need

That’s fine. Write a line about what is bothering you, such as sensitivity to cold, a chipped tooth or a wobbly molar, and we will help figure out the right kind of appointment. You do not need to know the dental term for it.

    Office hours and coverage

We are open Monday through Friday, 8:00 AM to 5:00 PM, and we follow up on requests during office hours. Mention your insurance or Oregon Health Plan (OHP) coverage when you write to us: we accept almost all dental insurance and will check your benefits before your visit.

    FAQ

Questions parents (and adults) often ask

How quickly will someone contact me?

We aim to respond as soon as we can. If your concern is urgent, please call (503) 822-0096 rather than waiting for a reply to the form.

Should I include my child’s medical history in the form?

No. Please keep private health information out of the form. We will go over medical history with you directly at or before the visit.

Can I book for my whole family?

Yes. List everyone who would like to be seen in the message, and mention any preference for coordinating visits on the same day.

Is it okay to call instead of using the form?

Of course. Call (503) 822-0096 and we can help you over the phone.
